This session showcases Medication Awareness Pathway Support (MAPS), an innovative, biopsychosocial model of care co-designed in Fleetwood Lancashire with both primary care and a specialist drug and alcohol service to deliver structured medication reviews, personalised reduction plans, and holistic support that considers patients’ mental health, lifestyle and goals.
The development of this pathway was led and evaluated by Health Innovation North West Coast. With strong evidence of impact, including significant reductions in opioid use and high levels of patient satisfaction, this session will walk attendees through how to genuinely co-design a patient-centred pathway, embedding lived experience, building trust with patients and adapting services in response to patient feedback.
The session will also cover how to deliver effective cross-system collaboration by aligning the interests of primary care, ICBs and local authority-commissioned services to address complex health challenges.
