Our heart failure national programme works collaboratively with partners and aims to improve the early detection, optimisation and long-term management of heart failure patients.
Where a local heart failure pathway is absent, Health Innovation North West Coast will help support places in identifying current patient pathways and developing one that utilises evidence-based approaches to manage patients, both through targeted and opportunistic case finding and the use of innovative diagnostics, as well as developing efficient pathways through pathway transformation and clinical service redesign.
