A shared-care pathway to support patients who are on high-doses of opioids has been co-designed by Inspire Lancashire (part of Change Grow Live), a specialist drug and alcohol service, Fleetwood (Lancashire) PCN, Lancashire and South Cumbria ICB, and Health Innovation North West Coast.

The pathway is called MAPS (Medication Awareness Pathway Support). It aims to provide safer, patient-centred reduction approaches through multi-disciplinary collaboration, pharmacist-led reviews, and access to psychosocial and recovery support.

As well as co-designing the pathway, Health Innovation North West Coast evaluated if MAPS:

  • achieved safe opioid reduction in patients on high-dose or multiple opioids
  • delivered a positive patient experience
  • was acceptable and feasible for clinicians and stakeholders.
