The Reimagining Care Pathways Collaborative Working Project (CWP) is a service offered by Novartis, which comprises a series of interviews and workshops, alongside a survey to engage a broader set of stakeholders. This work is taking place across Cheshire and Merseyside and focusses on baselining secondary prevention lipid management services. The CWP is a collaboration between Novartis, NHS Cheshire and Merseyside ICB and Health Innovation North West Coast.
The aim of providing pathway mapping support to Cheshire and Merseyside is to:
- Improve efficiency and service effectiveness for the benefit of patient care
- Address the local challenges they face in the delivery of patient care
The findings of this work will be outlined in a report and presented to NHS Cheshire and Merseyside ICB.
