The Innovation Agency, in partnership with NHS England, is delivering a transfer of care platform across Cheshire and Merseyside via which community pharmacists can better manage their patients’ medication post hospital discharge.

This training event, led by Hassan Argomandkhah, Chair of Pharmacy Local Professional Network (Merseyside), provided all community pharmacy staff an opportunity to find out more about TCAM and what steps they needed to take to provide your patients with a timely, safe and efficient service.

Feedback from previous event has been overwhelmingly positive, with a good balance of interactive presentations of the TCAM PharmOutcomes system and group work on case studies.
